Currently, the annual wellness visit labs that are covered by your yearly membership fee and subject to change from time to time include the following once each year at the time of your pre-wellness visit:

• Myeloperoxidase

• LipoProtein Fractionation NMR w/ Lipids

• ApoB

• Lp(a)

• Insulin

• HbA1c

• Vitamin D, 25 OH

• Comprehensive Metabolic Panel

• CBC w/ Diff

• TSH

• Lp-PLA2 Activity

• Microalbumin/Creatinine

All other labs drawn during the annual wellness or pre-wellness visit are sent to Quest and billed to insurance or directly to the patient.

For our male patients, if you have a urologist, please let the team know so your specialist can order your annual PSA.

As a reminder, separate from your annual MDVIP membership fee, all traditional services are billed to your insurance carrier (e.g., routine office visits, sick visits, injections, vaccines, urinalysis, strep and flu testing, standard physicals, EKGs throughout the year, etc.). You will be responsible for any coinsurance, co-pays, and deductible balances.  Additionally, services that are provided or processed by entities outside of our office, including Quest Laboratory, which has a phlebotomist stationed in our office, will be billed to your insurance carriers such as pap smears, PSAs, mammograms, and other laboratory and imaging services processed through Quest, other outside labs, imaging facilities, etc., and are not covered by your annual membership fee as part of the MDVIP yearly wellness exam.

We often have patients with lab orders from outside providers who want to know whether they can have those labs drawn in our office. Unfortunately, we are not legally allowed to draw labs ordered by outside providers in our office. Our Quest phlebotomist can only draw labs ordered by your MDVIP physician. Because of insurance regulations and prior authorization requirements, etc., you must have lab orders from outside physicians drawn at another lab, as we cannot have them drawn in our office. On this topic, please note that other than your annual wellness visit labs, which are included as a part of your MDVIP membership when drawn in our office to be sent to Cleveland Heart Lab, all other lab orders throughout the year may be sent to any lab you desire. Therefore, if you wish to have your routine lab orders from your MDVIP physician forwarded to an outside lab, please let the medical assistant know. We will be happy to send your lab orders to the lab of your choice, and we will provide you with a printed copy of your lab order(s) at check-out so you have them to take with you when visiting an outside lab.
